    Let me be the first to welcome you to the Bulldog Family! I understand change is never easy, so I wish to make this transition as stress-free as possible. If you have not already done so, I encourage you to explore our website to learn about the many academic, vocational, athletic, and artistic opportunities which await your child at South Fork High School. You will also be able to review the Florida graduation requirements which may differ from those of other states.

     

    Every journey starts with the first step...Please collect the necessary documentation and come see me to complete your child's registration. Appointments are not required. Please email me, at the email address listed above, with any questions.

    Your child will not be able to start school until all documents are received from the parent.

  • If, due to a loss of housing, you are forced to live in a shelter, motel, vehicle, or campgrounds, abandoned buildings, or doubled-up with relatives or friends; then you are eligible to receive services provided under the McKinney-Vento Act.

  • EXAMPLE OF INFORMATION YOU SHOULD BRING

    A withdrawal form from previous school, if registering AFTER school year has begun,
    must include school name, address and phone so we may request copies of any high school credits

    EXAMPLE OF TRANSCRIPT INFORMATION 
    Transcripts of credit earned (if possible)
    Documents relating to special education needs (IEP, NORA, etc.)

    INCOMING FRESHMEN FROM PRIVATE SCHOOL WE NEED PROOF OF 8TH GRADE PROMOTION AND FINAL GRADES AND EOC RESULTS.

    EXAMPLE OF PROOF OF BIRTH
    Acceptable Documents:
    State issued birth certificate
    Hospital issued record of birth
    Baptismal certificate
    Certified transcript of the record of baptism showing date of birth
    Valid passport


    Student's Social Security Number
    Not required - BUT if student applies for a scholarship or transfers to another Florida School - the SSN must be included on electronic transcript from SFHS

    Private and out of state students MUST provide evidence of physical exam within 1 year of enrollment

    PROOF OF RESIDENCY
    Must be for parent or guardian that student is residing with.
    Must include name of parent or guardian and be within the Martin County School District.
    Parent/Guardian must produce a valid State ID, or Driver's License with their current address.

    PLUS ONE acceptable document of proof of residence listed below


    Utility bill for service of residence (FPL, Cable, or Water)
    Rental or Lease agreement
    Contract to purchase/build
    Notorized affidavit (form 90D) along with FPL bill of owner - if living with other family members or friends.

    EACH STUDENT MUST FORMALLY APPLY AND BE APPROVED INTO A SIGNATURE PROGRAM.
    Form 407 may be picked up at any Martin County School
    Student must declare program at school of choice that they are applying to.
    Student MUST remain in the approved program to continue enrollment in the choice school.
    Student must be approved by program lead, principal and district
    School Choice may be revoked for attendance or discipline problems.

    ALL CHOICE STUDENTS ARE RESPONSIBLE FOR THEIR OWN TRANSPORTATION TO THEIR CHOICE SCHOOL.

    RECOMMENDATIONS ARE SUBJECT TO REVOCATION DUE TO THE CLASS SIZE REDUCTION STATE CONSTITUTIONAL AMENDMENT.

    THE SCHOOL CHOICE APPROVAL PROCESS TAKES TIME. YOUR FAMILY WILL BE NOTIFIED VIA EMAIL OR LETTER WHEN A DECISION IS MADE. IN THE MEANTIME, IT WILL BE NECESSARY FOR YOUR STUDENT TO CONTINUE THE REGISTRATION PROCESS AT THEIR HOME SCHOOL.
