As families gear up for a successful start to the new school year, the Florida Department of Health in Martin County is offering immunizations for school-aged children to help keep students safe and healthy.
Vaccines are available for children and adults who are uninsured, underinsured, or covered by Medicaid. The following immunizations are offered:
Hepatitis A & B
HPV (Human Papillomavirus)
Influenza (Flu)
Meningococcal
Pneumococcal
Tdap (Tetanus, Diphtheria, Pertussis)

Appointments are required. Please call 772-221-4000 (option 3) to confirm vaccine availability and schedule a visit.
Immunization Certification (DH 680 Form):
A DH 680 form (Florida Certification of Immunization) is required for school entry. Families should first contact their private healthcare provider to obtain this form. If your family is new to the area or you have not yet established a provider, the Department of Health can issue the form for a $10 fee.
