Locating Dental Providers
To find an in-network dentist, visit www.floridabluedental.com and select "search provider directory" From the drop down field under the "Find Doctors by Plan" section, select "Blue Dental Care Prepaid F" for the pre-paid dental plan or "Blue Dental Choice & Choice Plus (PPO)" for both the low and high dental PPO plans. Enter your determining search criteria in the appropriate fields.
Please note, under the pre-paid dental plan, employees and dependents must select at the time of enrollment an in-network dentist in the "Blue Dental Care Prepaid F" network to receive covered services. Primary dentists on file can changed by contacting Florida Combined Life directly at 877.325.3979. Those covered under the low or high PPO plans are not required to have a primary dentist on file with Florida Combined Life.
