Retiree Life Insurance

Retirees currently enrolled in the Martin County School District's Basic and Supplemental Term Life insurance may continue coverage with The Hartford. Employee Basic Term Life Insurance is subject to age reductions. Benefit amounts reduce by 35% of the original amount at age 65 and a reduction of 50% of the original amount at age 70. If the employee becomes totally disabled while insured and is under the age of 60, the employee may be eligible for a waiver of  premium. Further information on this can be found by calling the Risk & Employee Benefits Department. The Supplemental Term Life insurance amount if set at a benefit of $5,000 for retirees.

It is imperative to keep a current and updated Beneficiary Form on file with the Martin County School District Risk & Employee Benefits Department. The most current dated beneficiary form will always supersede previous forms on file. Beneficiary forms can be updated as frequently as desired by the retiree. Process for Filing a Retiree Death Claim

When a retiree covered under a District-sponsored life insurance policy passes, the Martin County School District's Risk & Employee Benefits Department must be notified. To contact the Risk & Employee Benefits Department, call 772.219.1200 x 30206.

Following the report of death, the Risk & Employee Benefits Department will remit appropriate employment and coverage information to the life insurance carrier, The Hartford. Once assigned to a claims adjuster, The Hartford will send a packet, inclusive of a beneficiary claim form, to all beneficiaries on file for the policy. The beneficiary should complete the beneficiary claim form and send it, along with any other requested information, back to The Hartford directly and in a timely manner.

The beneficiary on file can also expect a packet to be mailed to their address on file by the Martin County School District. This envelope with contain a blank W-9 form, in the event that benefit premium overpayments are owed. Benefit premium reimbursement is not applicable in all circumstances. The beneficiary should complete the W-9 with their personal information, not the information of the deceased retiree, and mail it, along with a copy of retiree's death certificate to:
